lockref: add ability to mark lockrefs "dead"

The only actual current lockref user (dcache) uses zero reference counts
even for perfectly live dentries, because it's a cache: there may not be
any users, but that doesn't mean that we want to throw away the dentry.

At the same time, the dentry cache does have a notion of a truly "dead"
dentry that we must not even increment the reference count of, because
we have pruned it and it is not valid.

Currently that distinction is not visible in the lockref itself, and the
dentry cache validation uses "lockref_get_or_lock()" to either get a new
reference to a dentry that already had existing references (and thus
cannot be dead), or get the dentry lock so that we can then verify the
dentry and increment the reference count under the lock if that
verification was successful.

That's all somewhat complicated.

This adds the concept of being "dead" to the lockref itself, by simply
using a count that is negative.  This allows a usage scenario where we
can increment the refcount of a dentry without having to validate it,
and pushing the special "we killed it" case into the lockref code.

The dentry code itself doesn't actually use this yet, and it's probably
too late in the merge window to do that code (the dentry_kill() code
with its "should I decrement the count" logic really is pretty complex
code), but let's introduce the concept at the lockref level now.

The extra new ABS_xx values resulted in ABS_MAX no longer being a

NVMe: Merge issue on character device bring-up

A recent patch made it possible to bring up the character handle when the
device is responsive but not accepting a set-features command. Another
recent patch moved the initialization that requires we move where the
checks for this condition occur. This patch merges these two ideas so
it works much as before.

tcp: properly increase rcv_ssthresh for ofo packets

TCP receive window handling is multi staged.

A socket has a memory budget, static or dynamic, in sk_rcvbuf.

Because we do not really know how this memory budget translates to
a TCP window (payload), TCP announces a small initial window
(about 20 MSS).

When a packet is received, we increase TCP rcv_win depending
on the payload/truesize ratio of this packet. Good citizen
packets give a hint that it's reasonable to have rcv_win = sk_rcvbuf/2

This heuristic takes place in tcp_grow_window()

Problem is : We currently call tcp_grow_window() only for in-order
packets.

This means that reorders or packet losses stop proper grow of
rcv_win, and senders are unable to benefit from fast recovery,
or proper reordering level detection.

Really, a packet being stored in OFO queue is not a bad citizen.
It should be part of the game as in-order packets.

In our traces, we very often see sender is limited by linux small
receive windows, even if linux hosts use autotuning (DRS) and should
allow rcv_win to grow to ~3MB.

Provide a kernel-doc comment documentation for the BQL helpers:

Similarly to how it is done for the other functions, the documentation

mlx5: remove unused MLX5_DEBUG param in Kconfig

This patch proposes to remove the MLX5_DEBUG kernel configuration
parameter defined in drivers/net/ethernet/mellanox/mlx5/core/Kconfig,
but used nowhere in the makefiles and source code.

This could also be fixed by using this parameter,
but this may be a leftover from driver development...

bnx2x: Restore a call to config_init

Commit c0a77ec74f295013d7ba3204dd3ed25fccf83cb4 'bnx2x: Add missing braces in
bnx2x:bnx2x_link_initialize' identified indentation problem, but resolved it
by adding braces instead of fixing the indentation. The braces now prevents a
config_init call in some cases, though it should be called regardless of that
condition. This patch removes the braces and fix the confusing indentation
that caused this mess.

tcp: fix no cwnd growth after timeout

In commit 0f7cc9a3 "tcp: increase throughput when reordering is high",
it only allows cwnd to increase in Open state. This mistakenly disables
slow start after timeout (CA_Loss). Moreover cwnd won't grow if the
state moves from Disorder to Open later in tcp_fastretrans_alert().

Therefore the correct logic should be to allow cwnd to grow as long
as the data is received in order in Open, Loss, or even Disorder state.

net: netlink: filter particular protocols from analyzers

Fix finer-grained control and let only a whitelist of allowed netlink
protocols pass, in our case related to networking. If later on, other
subsystems decide they want to add their protocol as well to the list
of allowed protocols they shall simply add it. While at it, we also
need to tell what protocol is in use otherwise BPF_S_ANC_PROTOCOL can
not pick it up (as it's not filled out).

tile: rework <asm/cmpxchg.h>

The macrology in cmpxchg.h was designed to allow arbitrary pointer
and integer values to be passed through the routines.  To support
cmpxchg() on 64-bit values on the 32-bit tilepro architecture, we
used the idiom "(typeof(val))(typeof(val-val))".  This way, in the
"size 8" branch of the switch, when the underlying cmpxchg routine
returns a 64-bit quantity, we cast it first to a typeof(val-val)
quantity (i.e. size_t if "val" is a pointer) with no warnings about
casting between pointers and integers of different sizes, then cast
onwards to typeof(val), again with no warnings.  If val is not a
pointer type, the additional cast is a no-op.  We can't replace the
typeof(val-val) cast with (for example) unsigned long, since then if
"val" is really a 64-bit type, we cast away the high bits.

HOWEVER, this fails with current gcc (through 4.7 at least) if "val"
is a pointer to an incomplete type.  Unfortunately gcc isn't smart
enough to realize that "val - val" will always be a size_t type
even if it's an incomplete type pointer.

Accordingly, I've reworked the way we handle the casting.  We have
given up the ability to use cmpxchg() on 64-bit values on tilepro,
which is OK in the kernel since we should use cmpxchg64() explicitly
on such values anyway.  As a result, I can just use simple "unsigned
long" casts internally.

As I reworked it, I realized it would be cleaner to move the
architecture-specific conditionals for cmpxchg and xchg out of the
atomic.h headers and into cmpxchg, and then use the cmpxchg() and
xchg() primitives directly in atomic.h and elsewhere.  This allowed
the cmpxchg.h header to stand on its own without relying on the
implicit include of it that is performed by <asm/atomic.h>.
It also allowed collapsing the atomic_xchg/atomic_cmpxchg routines
from atomic_{32,64}.h into atomic.h.

I improved the tests that guard the allowed size of the arguments
to the routines to use a __compiletime_error() test.  (By avoiding
the use of BUILD_BUG, I could include cmpxchg.h into bitops.h as
well and use the macros there, which is otherwise impossible due
to include order dependency issues.)

The tilepro _atomic_xxx internal methods were previously set up to
take atomic_t and atomic64_t arguments, which isn't as convenient
with the new model, so I modified them to take int or u64 arguments,
which is consistent with how they used the arguments internally
anyway, so provided some nice simplification there too.

fuse: drop dentry on failed revalidate

Drop a subtree when we find that it has moved or been delated.  This can be
done as long as there are no submounts under this location.

If the directory was moved and we come across the same directory in a
future lookup it will be reconnected by d_materialise_unique().

fuse: clean up return in fuse_dentry_revalidate()

On errors unrelated to the filesystem's state (ENOMEM, ENOTCONN) return the
error itself from ->d_revalidate() insted of returning zero (invalid).

Also make a common label for invalidating the dentry.  This will be used by
the next patch.

fuse: use d_materialise_unique()

Use d_materialise_unique() instead of d_splice_alias().  This allows dentry
subtrees to be moved to a new place if there moved, even if something is
referencing a dentry in the subtree (open fd, cwd, etc..).

This will also allow us to drop a subtree if it is found to be replaced by
something else.  In this case the disconnected subtree can later be
reconnected to its new location.

d_materialise_unique() ensures that a directory entry only ever has one
alias.  We keep fc->inst_mutex around the calls for d_materialise_unique()
on directories to prevent a race with mkdir "stealing" the inode.

vfs: check unlinked ancestors before mount

We check submounts before doing d_drop() on a non-empty directory dentry in
NFS (have_submounts()), but we do not exclude a racing mount.  Nor do we
prevent mounts to be added to the disconnected subtree using relative paths
after the d_drop().

This patch fixes these issues by checking for unlinked (unhashed, non-root)
ancestors before proceeding with the mount.  This is done with rename
seqlock taken for write and with ->d_lock grabbed on each ancestor in turn,
including our dentry itself.  This ensures that the only one of
check_submounts_and_drop() or has_unlinked_ancestor() can succeed.

We check submounts before doing d_drop() on a non-empty directory dentry in
NFS (have_submounts()), but we do not exclude a racing mount.

 Process A: have_submounts() -> returns false
 Process B: mount() -> success
 Process A: d_drop()

This patch prepares the ground for the fix by doing the following
operations all under the same rename lock:

  have_submounts()
  shrink_dcache_parent()
  d_drop()

This is actually an optimization since have_submounts() and
shrink_dcache_parent() both traverse the same dentry tree separately.

as we do it exactly once.

Restructure d_genocide() to do the killing on entering the dentry instead

openvswitch: Fix alignment of struct sw_flow_key.

sw_flow_key alignment was declared as " __aligned(__alignof__(long))".
However, this breaks on the m68k architecture where long is 32 bit in
size but 16 bit aligned by default. This aligns to the size of a long to
ensure that we can always do comparsions in full long-sized chunks. It
also adds an additional build check to catch any reduction in alignment.

vxlan: Fix kernel panic on device delete.

On vxlan device create if socket create fails vxlan device is not
added to hash table. Therefore we need to check if device
is in hashtable before we delete it from hlist.
Following patch avoid the crash. net-next already has this fix.

net: mvneta: properly disable HW PHY polling and ensure adjust_link() works

This commit fixes a long-standing bug that has been reported by many
users: on some Armada 370 platforms, only the network interface that
has been used in U-Boot to tftp the kernel works properly in
Linux. The other network interfaces can see a 'link up', but are
unable to transmit data. The reports were generally made on the Armada
370-based Mirabox, but have also been given on the Armada 370-RD
board.

The network MAC in the Armada 370/XP (supported by the mvneta driver
in Linux) has a functionality that allows it to continuously poll the
PHY and directly update the MAC configuration accordingly (speed,
duplex, etc.). The very first versions of the driver submitted for
review were using this hardware mechanism, but due to this, the driver
was not integrated with the kernel phylib. Following reviews, the
driver was changed to use the phylib, and therefore a software based
polling. In software based polling, Linux regularly talks to the PHY
over the MDIO bus, and sees if the link status has changed. If it's
the case then the adjust_link() callback of the driver is called to
update the MAC configuration accordingly.

However, it turns out that the adjust_link() callback was not
configuring the hardware in a completely correct way: while it was
setting the speed and duplex bits correctly, it wasn't telling the
hardware to actually take into account those bits rather than what the
hardware-based PHY polling mechanism has concluded. So, in fact the
adjust_link() callback was basically a no-op.

However, the network happened to be working because on the network
interfaces used by U-Boot for tftp on Armada 370 platforms because the
hardware PHY polling was enabled by the bootloader, and left enabled
by Linux. However, the second network interface not used for tftp (or
both network interfaces if the kernel is loaded from USB, NAND or SD
card) didn't had the hardware PHY polling enabled.

This patch fixes this situation by:

 (1) Making sure that the hardware PHY polling is disabled by clearing
     the MVNETA_PHY_POLLING_ENABLE bit in the MVNETA_UNIT_CONTROL
     register in the driver ->probe() function.

 (2) Making sure that the duplex and speed selections made by the
     adjust_link() callback are taken into account by clearing the
     MVNETA_GMAC_AN_SPEED_EN and MVNETA_GMAC_AN_DUPLEX_EN bits in the
     MVNETA_GMAC_AUTONEG_CONFIG register.

This patch has been tested on Armada 370 Mirabox, and now both network
interfaces are usable after boot.

ethernet/arc/arc_emac: Fix huge delays in large file copies

copying large files to a NFS mounted host was taking absurdly large
time.

Turns out that TX BD reclaim had a sublte bug.

Loop starts off from @txbd_dirty cursor and stops when it hits a BD
still in use by controller. However when it stops it needs to keep the
cursor at that very BD to resume scanning in next iteration. However it
was erroneously incrementing the cursor, causing the next scan(s) to
fail too, unless the BD chain was completely drained out.

ntb: clean up unnecessary MSI/MSI-X capability find

PCI core will initialize device MSI/MSI-X capability in
pci_msi_init_pci_dev(). So device driver should use
pci_dev->msi_cap/msix_cap to determine whether the device
support MSI/MSI-X instead of using
pci_find_capability(pci_dev, PCI_CAP_ID_MSI/MSIX).
Access to PCIe device config space again will consume more time.

NTB: NTB-RP support

Add support for Non-Transparent Bridge connected to a PCI-E Root Port on
the remote system (also known as NTB-RP mode).  This allows for a NTB
enabled system to be connected to a non-NTB enabled system/slot.

Modifications to the registers and BARs/MWs on the Secondary side by the
remote system are reflected into registers on the Primary side for the
local system.  Similarly, modifications of registers and BARs/MWs on
Primary side by the local system are reflected into registers on the
Secondary side for the Remote System.  This allows communication between
the 2 sides via these registers and BARs/MWs.

Note: there is not a fix for the Xeon Errata (that was already worked
around in NTB-B2B mode) for NTB-RP mode.  Due to this limitation, NTB-RP
will not work on the Secondary side with the Xeon Errata workaround
enabled.  To get around this, disable the workaround via the
xeon_errata_workaround=0 modparm.  However, this can cause the hang
described in the errata.

tuntap: orphan frags before trying to set tx timestamp

sock_tx_timestamp() will clear all zerocopy flags of skb which may lead the
frags never to be orphaned. This will break guest to guest traffic when zerocopy
is enabled. Fix this by orphaning the frags before trying to set tx time stamp.

The issue were introduced by commit eda297729171fe16bf34fe5b0419dfb69060f623

tuntap: purge socket error queue on detach

qlcnic: use standard NAPI weights

Since commit 82dc3c63 ("net: introduce NAPI_POLL_WEIGHT")
netif_napi_add() produces an error message if a NAPI poll weight
greater than 64 is requested.

qlcnic requests the weight as large as 256 for some of its rings, and
smaller values for other rings. For instance in qlcnic_82xx_napi_add()
I think the intention was to give the tx+rx ring a bigger weight than
to rx-only rings, but it's actually doing the opposite. So I'm assuming
the weights do not really matter much.

Just use the standard NAPI weights for all rings.
